He then moved to Fort Campbell and spent four years as a staff physician at Blanchfield Army Community Hospital (BACH). During his service at BACH, Dr. Pike was deployed to Afghanistan and was honored with the Bronze Star medal for his service. After Dr. Pike was honorably discharged from his U.S. military service in 2007, he then moved onto the Tennessee Department of Health where he worked as Medical Director at the Stewart County Medical Center in Dover, Tennessee. Dr. Pike’s most recent medical position was as a Staff Physician at the Screaming Eagle Medical Home (Gateway Medical Center).

Jacqueline Pike

Jacqueline Pike is the Practice Manager at Elite Primary Care, where she oversees daily operations and works closely alongside her husband, Dr. Carlo Pike, to ensure the clinic’s vision of compassionate, patient-centered care is fulfilled.

A graduate of Aurora University with a Bachelor’s degree in Social Work, Jacqueline brings a unique blend of empathy, organization, and leadership to her role. Her background in social work gives her a deep understanding of patient needs and effective team collaboration, which she applies daily to create a positive and efficient clinical environment.

After dedicating 12 years to homeschooling their children, Jacqueline returned to the workforce with a renewed sense of purpose. She takes great pride in managing the administrative and operational aspects of Elite Primary Care, helping the practice grow while maintaining its high standard of excellence.

Jacqueline has been married to Dr. Pike for 29 years, and together they continue to build a family-centered medical practice that values integrity, quality care, and community trust.

Shannon Horstman, PA-C

Shannon Horstman, PA-C, is from Clarksville, TN. She attended Austin Peay State University where she majored in Biology with a minor in Pre-Professional Health. She graduated with a Master’s Degree in Physician Assistant Studies from Trevecca Nazarene University in Nashville, TN. She is certified through the National Commission on Certification of Physician Assistants.

Shannon has experience in Emergency Medicine, OB/GYN, and Primary Care for pediatrics and adults. Her top priority as a Family Medicine provider is to ensure each patient receives the best, evidence-based medical care for the management of acute and chronic conditions.

Katherine Shelby, FNP-C

Katherine Shelby, FNP-C, is from Lexington, TN. She attended Jackson State Community College where she received her Associates Degree in Nursing. She later received her Bachelor of Science in Nursing degree from Western Governors University. After working for several years as a Registered Nurse she graduated with her Masters Degree in Nursing with a Family Nurse Practitioner concentration from Austin Peay State University. She is certified through the American Academy of Nurse Practitioners.

Katherine has experience in gastroenterology, neonatal and pediatric populations, and Primary care for pediatrics and adults. Her goal is to provide compassionate, patient-centered and evidence-based care that addresses the physical, mental, and emotional needs of the whole person, serving as a lifelong learner and a dedicated advocate for her patients.

Katherine lives in Robertson county with her husband and daughter.
